There is no need to complete this form multiple times. We only require this information once off and can use the same parent/guardian's approval for all other tours booked for the same minor.

I, the undersigned and “legal guardian” or “parent” acting on behalf of the minor herein identified, give the “minor” permission to partake in the Hotspots2c tour/s I paid & signed for. I am aware that the Hotspots2c tour and/or associated activities which the “minor” will undertake, could have inherent dangers. In spite of the fact that every effort is made to ensure safety, the tour/s are undertaken at the participant’s own risk, and the participant is responsible to abide by the following terms and conditions of operations and indemnity:

Indemnity Terms & Conditions:

* I understand that every person participating in the activities during a Hotspots2C tour will do so at their own risk, and will not take any legal action against, and to indemnify, hold harmless and absolve Hotspots2C, its owner, guides and agents from claims and or liability in connection with loss or damage to property, injury or death, arising from any accident or negligence of the above parties, or from any cause whatsoever during an activity on the tour.

* I understand that I am obliged to notify Hotspots2C of any medical conditions that might affect the minor during any activity while on tour with Hotspots2C so that special precaution can be taken beforehand. When participating in any activity while on tour with Hotspots2C, I hereby declare that the minor is fit and in good health, and is prepared to act responsibly at all times and participate at their own risk.

* I also undertake to pay all the costs of medical rescue and or medical treatment and transportation and evacuation in case of an emergency.

* I freely and wilfully agree to all the terms and conditions under which any activity is being conducted.
* The minor will agree and is required to obey all the instructions given by Hotspots2C guides at all times.
