Use this form to submit formal nominations for California Rare Plant Rank (CRPR) additions, changes, deletions, and/or Threat Rank and taxonomic name changes in the CNPS Rare Plant Inventory (RPI). Information requested on this form is intended to support the rationale of the addition or change proposed. Please submit separate forms for each proposal you have.

All information submitted on this form, except for personal contact and sensitive plant location information, may be made available to the public upon request.
