Proposal form for NJHS Individual Projects for 2015-2016.  Make sure that you have a personal copy of all of the information that you include here!  You might want to create a Word or Google Doc for yourself, and then cut and paste your answers in the appropriate boxes on the form.  Most questions are required, but a few are optional depending on your project.

Include first and last names

Project Proposal *
Include a brief description of your project proposal.  Make sure that you focus on how your project will benefit the community.
Proposed Project Timeline *
Include major approximate dates (or anticipated dates) for different elements of your project.  Be as specific as possible, although we recognize that these dates may change.
Project Funding *
Will your project require funding?  If so, explain how you will raise money or obtain funding for your service project.  Be as specific as possible!  If you will not require funding, write N/A in the space provided.
Evaluation of Sustainability *
How will you ensure that your project is sustainable?  Give specific details explaining how your project will continue after you have started it.  Are there outside sources of funding, etc, that will keep the project going?  Be specific.
Project Collaborators
Include the names of any students with whom you will be collaborating on this project.  Also indicate which aspect(s) of the project each of you will be in charge of.
