1. 「精益創業於善」的截止報名日期為6月25日。計劃費用為港幣$3,000。於5月29日前報名將有港幣$500元折扣。請填妥以上表格後5個工作天內，以下列方法繳付課程費用﹕

銀行過數﹕請將費用存入仁人學社有限公司​​之香港上海匯豐銀行戶口 [023-133176-838]，然後把銀行入數紙電郵至ino@education-for-good.com，入數紙上請寫上姓名、聯絡電話及計劃名稱。
Bank transfer: Please settle the payment through bank transfer to “Education For Good Community Interest Company Limited” (HSBC Bank A/C: 023-133176-838). Please mark you​r​ name, contact number and ​program name on the bank-in​-​slip and ​​email​ ​to info@education-for-good.com.

郵寄支票：郵寄劃線支票，抬頭請寫 [仁人學社有限公司]，寄回香港西九龍通州街500號星匯居L1［仁人學社有限公司收］，支票背面請寫上姓名、聯絡電話及工作坊名稱。
By cheque: Please ​make a crossed cheque payable to "Education For Good Community Interest Company Limited". Please ​mark you​r​ name, contact number and ​workshop name at the back of ​the ​cheque and mail to "Education for Good​ ​Community Interest Company Limited"​ at​ L1, The Sparkle, 500 Tung Chau Street, West Kowloon, Hong Kong.

2. 工作坊更改
如報名人數不足，仁人學社有權取消該計劃。

3. 退款
除非因工作坊額滿或取消，否則已繳之費用恕不退還，退款將會在四至五星期內以支票形式寄還給申請人。

1. 此報名表格內所提供的個人資料將供仁人學社處理報名之用 。如工作坊申請不被接納，仁人學社將銷毀一切有關的個人資料。

2. 此報名表格內所提供的個人資料將供仁人學社向申請人推廣最新資訊，包括任何舉辦之活動、開辦之課程、各項優惠及服務有關之活動。這些個人資料不會轉交予仁人學社以外的其他機構作上述用途。

3. 根據個人資料(私隱)條例，申請人有權查閱及更改其個人資料。申請人如需更改其個人資料，請直接聯絡仁人學社以作安排。
