2. The form only needs to be filled out once, per child, for the 2026-27 school year.

5. Medical insurance information is needed.

6. A physical examination taken April 1 and thereafter is valid for the following two school years. A physical examination taken prior to April 1 is valid only for the remainder of that school year and the following school year. A current Physical Examination Form must be on file with the Athletic Department (email a photo to schererm@wawmsd.org or drop of the form in the athletic office). Here are some recommended clinics should you need to schedule a physical exam.

7. Athletic payments can be made online through your family's Skyward account, or with cash or check in the athletic office. Student-athletes' payment will be posted on the account one week after final rosters are submitted to the athletic department. Payment must be made at the start of the season for your student-athlete to be eligible to compete. You will receive notification via email when fees are assessed, and if you have any unpaid fees. Students with unpaid fees from previous seasons will not be able to participate until the fees are paid.

8. Students are not eligible to participate in athletics practices or games until this online registration form is completed and an updated physical is on file. Emails will be sent to all interested athletes and families informing you of your status.
