Dear Students,

After making your requests in person, this form should be submitted to your two teachers in the second semester of junior year or before September 1st (at the latest) of your senior year.

You will also need to officially request your recommendation letters via Scoir by the end of junior year or by September 1st (at the latest) of your senior year. In Scoir, go to "My Colleges" and click on "Docs" in the top right of the screen, scroll down, and choose your teachers' names from drop down menu.

STEPS FOR COMPLETING THIS FORM:

1. Complete and submit form

2. Check your email for copy of your responses

2. Forward the copy of your responses to your teacher's email

Teachers upload their recommendations to your Scoir accounts by October 15th of the senior year. If you have any questions, please talk to College Counseling.
