I give my child permission to participate in the ETM-LA Instrumental Program. I understand that they are encouraged to attend every class, practice, and bring their instrument and will encourage my child to complete the required work for the course. Education Through Music-Los Angeles: Media Waiver & Consent Form (Optional) | Exención de medios y formulario de consentimiento (Opcional)

I understand that Education Through Music-Los Angeles (“ETM-LA”) is a nonprofit organization dedicated to partnering with schools to encourage music education. I grant to ETM-LA the right to use the image, name, photograph, voice, persona, and/or likeness of myself, my child, or the child for whom I am a legal guardian (collectively, the “Media”) in its promotional materials and publicity efforts. In addition, I grant to ETM-LA the right to use materials created by my child for their music teacher (also collectively, the “Media”) in its promotional materials and publicity efforts. I understand that any such photographs, recordings, and other video or audio may be used in publications, print ads, electronic media (e.g., video, Internet, social media, marketing collateral) or other media. I waive the right to inspect or approve any materials incorporating the Media, as well as any right to royalties or other compensation for the use of the same. I also waive any claim to any intangible rights, including the right of publicity or copyright that I or the child for whom I am the parent or legal guardian may hold concerning the Media created by ETM-LA. I release ETM-LA from any claims or liability that I or the child for whom I am the parent or legal guardian may have arising out of the recording, use or reproduction of the Image. | Entiendo que Education Through Music-Los Angeles (“ETM-LA”) es una organización sin fines de lucro dedicada a colaborar con las escuelas para fomentar la educación musical.
