Please complete this form to request your Cape Elizabeth High School transcript. This form is not for current CEHS students. Transcripts can be mailed, faxed, emailed or picked up in person at CEHS.

Please note that official transcripts are embossed and mailed with a seal, that if broken, may invalidate the transcript for official purposes. Please consider this when requesting a transcript for a third party, as most require an institution-to-institution delivery.

Transcripts can take 2-5 school days to process. During the summer recess, transcript requests are processed once a week.
