TMDC 2025-2026 School Year Contract Agreement & Waiver

Thank you for choosing us to be a part of your child’s dance training. Please read and fill out this form in its entirety. This will be used as our formal registration contract agreement for our 2025-2026 school year.

Please fill out this section if you feel there is important information about the dancer we should be aware of. Example: any medical, physical, psychiatric, emotional, and or behavioral conditions.

Monthly Tuition

Tuition is due within the first five (5) business days of each month. 
Accepted payment methods: Cash, Zelle, CashApp or Credit/Debit card (3% fee applies). A $10 late fee will be added if payment is not received by the deadline.

Card on File

A valid credit/debit card is required to be kept on file. 

If tuition is not paid by the deadline, your card will be automatically charged for: Monthly tuition + $10 late fee + 4% service charge.


Please note: Your card will NOT be automatically charged monthly. You are responsible for submitting tuition payments on time.

Refunds & Delinquent Accounts

All tuition payments are non-refundable. 

Tuition is paid month-to-month (no yearly contract). 

Students will not be permitted to take class if their account is 30+ days delinquent. 

If one month’s tuition is missed, the registration fee will be reapplied.

Holidays

Tuition does NOT adjust for holidays. 

Private Classes

Must be paid at the time of scheduling. No-shows will not be refunded.

Attendance Policy: 
-Steady attendance is a requirement for success. 
-Parents/Guardians are responsible for communicating  absences to the dance studio office. -Missed classes CANNOT be made up. 
-There are NO REFUNDS on missed classes. 

Schedule Changes or Dropping Classes:
-The office must be notified prior to changing a class or discontinuing a class.
-If the office is not notified, the card on file will be automatically charged after the five business day mark.
-All fees are due until the office is notified of any classes being dropped.
-No tuition will be refunded when dropping a class.

Personal Items: Please do NOT send your children with any important OR valuable items. We are NOT responsible for personal property.  *
Addressing Issues:
-Concerns may be addressed with the director/owner outside class time. 
-Do not confront other parents or children at the studio. 
-No tolerance for angry dialogue, foul language, or threats. 
-No refunds if contract is terminated. 

We reserve the right to refuse service to anyone.

End of the year Recital: (optional)
-We will be having an end of the year recital. This is to offer something our students can look forward to at the end of the year. 
-Payments will made throughout the year to make it easier to secure your spot. 
-We will be giving out more information throughout the school year.

Photo and Video: 
I grant permission to The Movement Dance Center to use the name and likeness in photography and/or video of my child(ren) for advertising and promotional purposes without compensation to my child(ren) or myself.

Studio Rules for Parents: 
-Anyone who is within the facilities must have good conduct and address in a respectful manner with all staff.
-Adults must be responsible for their children. No running, yelling or rough playing within the facilities
-Parents must wait in the lobby. 

Observation of Classes: 
-In order to maintain the comfort, privacy, and productivity of our classes we do NOT allow parents to watch during class time. We will always allow you to watch on observation days or sometimes the last 5 minutes of class. 
-Instructors will decide how often classes can be observed. 

OUR GOAL IS TO ENSURE THE PRODUCTIVITY OF THE CLASSES WITHOUT ANY DISTRACTIONS.

Uniform:
-The dancers must wear their proper uniform assigned to each class everyday. 
-Technical classes (ballet, jazz, lyrical, technique) uniform consists of the black TMDC leotard, black tights/leggings, and the appropriate dance shoes (ballet slippers or half sole shoes).
-Hip Hop classes t-shirt, tights/ joggers/ warm-ups, and tennis shoes.  

HAIR MUST BE PICKED UP FOR EVERY CLASS. 

NO CROCS FOR HIP HOP PLEASE!!!

Contract and Release of Liability:

I acknowledge that participating in this activity involves risks, including those from negligence or defective equipment. I confirm I am in good health and have no issues preventing my participation.

I release The Movement Dance Center and its affiliates from any liability for injuries, illnesses, or accidents during or related to the activity. This includes any claims arising from negligence or other causes.

I agree not to sue and to indemnify the mentioned entities against any claims or liabilities. I understand this is a legal contract and I sign it voluntarily.

For Minors: If the participant is under 18, I, as the parent or guardian, consent to their participation and agree to these terms on their behalf.

IMPORTANT Dance Etiquette: 

Our studio expects all dancers to show respect to themselves, peers, and instructors while learning proper technique and work ethic. Students must arrive on time for warm-up, which is required to prevent injury. Dancers should bring water or money to purchase it at the studio.


The following rules will be enforced:
-Proper dress code is required for all dance classes.
-Hair must be pulled up.
-NO jewlery of any kind.
-Cell phones MUST be turned off/silenced before class.
-NO smartwatches
-NO gum is allowed in class.
-NO bullying. 
-Be respectful and kind to instructors, peers, & staff. 
-Uncontrollable behavior that disrupts class will NOT be tolerated, and student contract will be terminated. NO REFUND will be issued.
