Private Party/Event Interest Form 
Thank you for your interest in my Permanent Jewelry Private Party/Event. If you are interested please fill out the form. All submissions will be reviewed carefully and you will receive a reply within 2-3 business days via DM on IG. For any questions do not hesitate to message me directly.  
Our goal is to make every event memorable for your special day with friends and family. If you have any special requests, please let me know. All events must be booked at least 4 weeks in advance.  
For private events/parties, there is a $100 non-refundable deposit that will be applied towards the total balance. A minimum of 8 people or 8 pieces of jewelry is required to book. The host will receive a FREE bracelet as a gift from me. For 15+ guests, discounts may apply.  
 
Travel fee: $30+. This applies to areas that are 15 miles outside the Claremont area. Depending on the distance of the event, the fees increases. If cancellation is needed, we require a 15 day minimum to avoid the loss of the deposit.
