This year we need all families to identify two adults who will be supporting their child's separation. Please complete the form below by Monday the 10th of August. Identification cards will be created for each of these adults to allow them entry onto campus with your child.

Once we have this form a health declaration form will be sent to you for each of the two identified adults. Without this form the card will not give you access to the campus.

Please send your headteacher a photo of each of the adults identified to create the ID card.
