Club Title *
Provide a title for your club. It can be simple and informative (like Yoga Club) or interesting and different (like Winter Wiggles!). Please make it short.
Club Description *
Describe what the club is about. You can provide details of themes or activities you already have planned for each week, or just describe briefly what will the kids do in general. Make it exciting!

Club Volunteer
Name of the person assisting the leader. If parents volunteer for this role, their kids will be in for free. If you don't have a parent volunteer yet, leave this empty and we will find you one.

Duration (in weeks) *
How long will your club last? Typically clubs last from 4-6 weeks, but you can make it shorter or longer if you wish
Day of the week *
Choose the day(s) of the week that works for you. If you choose more than one, we will assign you the day that works best for the overall schedule of clubs this session. Clubs run on weekdays after school starting at 2:45.

Pick the consecutive grades you'd like to work with. We often have clubs K-2 and 3-5, but you can customize it.

Number of Kids Limit *
How many kids (max) can you have in your club? - including your own Missituk child(ren) (don't count non-Missituk siblings). Keep in mind you will have a parent volunteer there to help you, so will not be alone.
We recommend 10 kids. You could do anywhere from 8 - 14 kids, but more than that will need additional volunteers.
Cost *
How much will the club cost for each kid? Consider the cost of materials and stipend for you (optional). Feel free to e-mail clubs coordinator if you need help with this. The minimum is $5.
Room requirements
Depending on the number of clubs we are offering at the same time, we can have a few rooms available for you to use. Please provide a description of the kind of room you will need (parent room ok? gym? music room? art room? computers or iPads? outside? big enough for something in particular, etc)
Specific date requirements
Is there a date that doesn't work for you? We can skip dates if we know in advance, or we can start your club a little later. Ideally, we'd like all clubs to start the same week to avoid confusion, but we are flexible if needed.

Let us know any other things that you'll need to run a successful club. Special materials, tools, equipment, etc
