(TTCAS), its directors, employees, instructors, agents, successors and assigns from any and all claims, causes of action, suits or liability, cost and expenses (including without limitation reasonable attorneys’ fees) arising out of, or in connection with, any loss, personal injury, death, or other damage that I may incur or suffer by reason or as a result of my participation in any of TTCAS' programs. I have been fully informed about the nature of martial arts, dance, or sports training, including its potentially hazardous aspects, and am fully cognizant of and specifically assume all risks attendant in participation.I, the student or member, irrevocably authorize the school, its successors and assigns, and those under its authority, to copy, use, published for art advertising, trade or any other lawful purpose whatsoever, photographic portraits or videotapes of me, in which I may be included whole or in part. Being a guest, I will faithfully comply with all the rules and regulations of TTCAS. Failure to take lessons in the allocated time, without written consent or approval, invalidates the lessons beyond the expiration date and does not relieve me of my obligation to pay tuition in full if applicable. I have read this Waiver of Liability form in its entirety, have had it explained, and fully understand its significance and effect. Three Treasures Cultural Arts Society, its directors, employees, instructors, or agents have executed this Waiver of Liability form voluntarily and without reliance upon any statement.
