This form is to assist in gathering information for us to call you for setting up a preschool evaluation for students ages 3 to 5, though we can take your information over the phone at (435) 283-4171. Our schools are primarily funded from Special Education but we do take some tuition students. Providing your student's information to this form will get your child on the evaluation list for the next available testing date or for consideration for one of the tuition spots. Special Needs Opportunity Scholarship Grants
The Carson Smith Special Needs Scholarship provides tuition assistance for qualified students with disabilities enrolled in eligible private schools. The scholarship is for students who would qualify for special education and related services in public schools, preschool through 12th grade (3‐21 years of age), whose parents choose an eligible private school. The Special Needs Opportunity Scholarship Program is an education choice model for Utah residents that provides tuition assistance for eligible students with disabilities not enrolled in public school. It is designed for students who would qualify for special education and related services in public schools, kindergarten and grades 1 through 12 (5–21 years of age), whose parents choose an eligible private school or other eligible expenses.
