Please complete the questions on this form to register your child/children for our Aftercare Program. There is a ONE TIME Registration Fee of $30 per family that will be automatically deducted through your FACTS account upon submission.

Monthly statements will be sent out by the 15th of each month. By checking the box below, you agree to have your full payment deducted automatically each month through FACTS to cover the cost of your child care, and the one-time registration fee of $30.
