By filling out this form, you are requesting a YES Card for an eligible student. Please note:
- All 6th-8th grade students at Second Street School, Bondurant Middle School, and Elkhorn Middle School are eligible for a Yes Card. 7th & 8th grade students at The Academy are also eligible.
- Regardless of the expiration date on the card, all funds must be spent by July 30, 2024. Any unused funds will be removed from the card on July 31, 2024 and used towards eligible students’ Yes Cards for the following school year.
- Please DO NOT share this link with anyone else. Sharing the link with others can lead to confusion and disappointment for those who are not eligible. Thank you for your understanding.
- If you have more than one child eligible for a YES Card, you will need to fill out a form for each child. Once you have submitted the form for the first child, you will have the option to submit another response.
- There will be NO custom orders this year. The YES Card can only be used at participating providers. The latest list of providers can always be found at http://bit.ly/yescardupdates.
The process for issuing cards will be different at each school. Please allow up to one month to receive the card from your school or from us. If you have any questions or concerns in the meantime, please contact our Program Coordinator at ellery.denny@ky.gov. Thank you!
