Enrollment paperwork must be completed/submitted at least 2 weeks prior to your start date. This paperwork includes health forms that must be signed by your child’s pediatrician, as well as forms that must be signed by the physician of the caregiver who will be working in the classroom. These health forms typically require a physical exam by a physician, though some physicians might complete the forms without one, as long as there is one on file within the last calendar year. Making an appointment with a physician can take several weeks, as can submitting the paperwork for completion. SCPNS recommends giving yourself at least a month to complete all health forms, so please plan accordingly.
