This name will be used in the GRADEpro instance for organization setup and will also be used in the license (unless we are instructed otherwise)
Country (of your organization) *
This determined which Evidence Prime branch will issue the invoice for your license.
Billing address *
This address will be used to issue the invoice. Please include the name of the organization if different from the one above. Please include VAT-EU number of EU VAT-registered organizations.
Number of projects *
1 by default. Each active project costs 2400 USD per year
Project start date *
The intended date when your license starts. The project setup takes about 1 day. The license period can start before the actual payment (we use NET30 payment terms by default).

Payment currency *
This will determine the currency on the invoice and the bank account for the wire transfer. If non-USD is used, the amount on the invoice will be recalculated from USD using the current exchange rate published by Google.
Means of payment *
Sorry, we do not accept cheques. The wire transfers: CAD transfers to a bank in Canada, other currencies to a bank in Poland. Credit card payments will be processed by PayPal.
