DONATION REQUESTS MUST BE RECEIVED AT LEAST 6 WEEKS BEFORE THE DATE OF THE EVENT.
Gift certificates will be sent by email.
This form must be completed in its entirety. Once you have done so, please inform our marketing department at firstname.lastname@example.org. Please note that you will not receive a response if you do not submit confirmation that you have completed the form. If the request is approved, you will be notified.
Submission of the form does not guarantee a donation. Our marketing team meets once a week to review all requests, but due to executive travel schedules, the meeting may not take place for several weeks.
