EMS Djapo Cultural Arts Institute 6 grade Permission Slip 2024

Djapo Cultural Arts Institute will provide cultural arts after school programming to 6 grade students for a duration of one month twice a week on Mondays' and Wednesdays' beginning Monday October 14, 2024 and ending Wednesday November 6, 2024 from 3:00-4:30 pm. This program will be held at the EMS in the cafeteria. The final rehearsal date and the culminating activity date - upon approval - will be announced to the parents so they can attend. 

The school WILL NOT provide transportation home for your child. Your child MUST have transportation home from the program. The program ends at 4:30 pm. 

If you are interested in having your child participate, please complete and submit the 
permission slip below. The DEADLINE to fill out the form is Monday October 14th, 2024 (End of Day). 
 We will ONLY accept the first 30 students whose parents sign them up. Students must have their permission slip signed and returned before they can attend.
