This form is required of all drivers/chaperones for day and overnight field trips or events. The contact information and acceptance of the Mission and Doctrinal Commitments is required of all volunteers.
Drivers and Chaperones will be sent a link from Paycom where you can directly enter your information securely to complete a motor vehicle background check. It is possible that the email from Paycom may go to your spam or junk mail folder, so please check. This must be done a minimum of 3 days prior to the event in order to be approved to drive/chaperone.
Drivers of personal vehicles will also be required to complete the vehicle insurance and information page.
