Yale Public School District Lunch Request
Please Note: This will be for the December 4th pick up.


Dear YPSD Families,

As we did last spring, we are happy to provide breakfast and lunch items for our students during this pause form face to face learning. All YPSD families are welcome and encouraged to sign up if they so choose. To ensure we can prepare enough breakfasts and lunches for students, we request that families complete the following form each week. Students will receive five breakfast and lunch items for the upcoming week. Food items for the week can be picked up at marked locations outside Yale High School, Avoca Elementary, and Farrell Emmett Elementary. (All pick up locations will be near the school gyms.)

Yale High School, Yale Junior High, and Yale Elementary families can pick up their items on Friday from 11:45-12:15.

Avoca Elementary and Farrell Emmett Elementary families can pick up their items on Friday from 12:30-1:00.

For our record-keeping process, we ask that you complete the form for each child who attends Yale Public Schools. Requests will need to be submitted by Wednesday for pick up on Friday.
