THIS INSURANCE POLICY APPLIES TO STUDENTS IN GRADES 6-8 ONLY (Middle School)

STUDENTS IN GRADES K-5 HAVE DEVICES UNDER MANUFACTURER WARRANTY: You do not need to submit this form or payment for students in Grades K-5.

Should payment not be made to cover uninsured devices, students may not be issued new, or temporary devices, at the discretion of the IT Department. *
