Synagogue Breastfeeding Friendliness Questionaire
These questions will help share information about the experience of breastfeeding mothers in synagogues and minyanim around the world with others, to help new mothers and visiting parents prepare best for a new synagogue.  

The information will be shared at http://www.nursingatshul.wordpress.com  It will not have any name associated with it (I won't even know who filled out the questionnaire), and generally I will paraphrase the information that you give me.  I do sometimes share quotes- if you would rather that I not do so, please let me know in the very last ("anything we missed") space at the end of the questionnaire.  If/when I receive more than one questionnaire about a particular synagogue, I will share any conflicting or differing positions on the website.  

This questionnaire was prompted by my own experiences, and is not associated with any organization.  I may use the information collected, in aggregate, to inform me and eventually the leadership of synagogues about what practices are common, appreciated, hurtful, etc.
