Request for Honorarium for Presenting PD Workshop 

This is a form for anyone who presented at a Pro-D event in SD48. This is not for teachers who are participating in Pro-D workshops. There is no honorarium available for teachers who participate in PD outside of school days/hours. 
 
Instructions:
1) Please complete this form after you have presented.
2) Forms submitted to the SSTA are reviewed by the Pro-D chair who will then provide reimbursement.
