Hum. 11 End of Year Evaluation Form
It has been a fun and rewarding experience working with the class of 2014.  From camping in the desert, to putting Fred Korematsu on trial at the courthouse, from 80's dance parties to learning coping mechanisms for dealing with the stress of junior year, each of you have taught me immensely and I am grateful to have had you in my class.

It is important for me to always be growing and improving as an educator and to offer a challenging yet engaging learning experience for all of my students. Thus, I'd like to offer you the opportunity to reflect on the past year and evaluate your experience in Humanities 11.  Please provide thoughtful, sincere and helpful feedback as I will most certainly read ALL of it and incorporate major trends into my curriculum planning for next year.

Your name is optional, but it is my philosophy that if you want to see positive change, you have to own it and I thus encourage you to identify yourself and provide feedback for which you are willing to take accountability and allow me an opportunity for non-hostile, totally friendly follow-up with you should I have any questions about your suggestions.
