*Age grades match BBNZ age groups. For example if player turns 14 in 2026 they are ineligible to play in the under 14 grade. If player turns 14 in 2026 they are too old for under 14 grade and must register for under 16 grade.

If you have one or more children, please complete a separate form for each child/rangatahi/adult.

Please READ CAREFULLY before completing this form:

- We can only take teams away who we have a coach for, therefore we are unable to cater for all grades.

- Our teams will have no more than a maximum of 10 players. Some teams will have 8 or 9 players. This will be at the coaches discretion.
