OFFICIAL PROXY FORM
Please read carefully before assigning your proxy vote.

TWGGA By-laws: Director Proxies: 4.16 A director may vote by proxy. All proxies must be in writing, must bear the signature of the director giving the proxy, and must bear the date on which the proxy was executed by the director. No proxy is valid after eleven months from the date of its execution.

IMPORTANT REMINDERS;
1.  If you are Director and are unable to make the meeting via video/phone, you may assign your proxy to another Director member.  Only 1 proxy may be accepted by each Director.
2. Before you enact a proxy for a specific meeting, please confirm with the Director that they can attend in your stead.
3. You will need to confirm with the Director that they DO NOT already carry a proxy, a Director member can only hold ONE proxy for each meeting.
4. We must have your proxy in writing within 24 hours of the meeting.
