One form is to be filled out for EACH request.

Official Transcripts can only be sent directly to a college or university. They are NOT given to Alumni.

Unofficial Transcripts can be given to Alumni for their personal use.

There is a $3.00 fee per transcript copy. If writing a check, make is payable to:
