The deadline to request the use of the Local School Wellness Policy Triennial Assessment Waiver was on June 30, 2020. Per USDA guidance we are unable to accept requests to use this waiver after this deadline. As this date has passed, you should attempt to complete the assessment as soon as possible.

Please know DPI understands the burden the COVID-19 pandemic has placed on your SFA and appreciate that this may not be the most pressing concern at the moment. That said, failure to complete the triennial assessment will be viewed as a finding requiring corrective action as part of the Administrative Review process. SFAs are required to use the Wisconsin Local Wellness Policy Triennial Assessment Report Card to fulfill the triennial assessment requirement.
