See the SSNL Manual for full requirments and exceptions to Student Eligibility to make sure you meet all criteria before filling out this form. This form must be submitted no later than the registration deadline of the Qualification Tournament listed on the SSNL Athletic Calendar for the sport in question in order to compete in the Qualifier Tournament.

The Eligibility Pathway for player selection will follow

i. Players in your own school, male or female (7-12)

ii. Players in a neighbouring school (10-12)

iii. Players in a neighbouring school (7-9)

Approvals will be made on a case by case basis following all eligibility rules including and not limited to SSNL's 80/20 rule

IMPORTANT: This form should be completed by the school requesting permission for their students to play for another school. This should NOT be submitted by a school requesting to pick up students from outside their school.

Students/school are not permitted to select which school they wish to play for. Upon receipt of this form, your SSNL Regional Director will investigate which school in the region has the most need and can also accommodate your player and place students accordingly.
