The Lacrosse Club of Montclair                     Financial Assistance and Scholarship Form 2020/21
Each year we are delighted to award scholarships to our players.  Scholarships are available through the generous donations made to our lacrosse club.  We strive to provide as many scholarships as possible to ensure that LCM fees do not deter players from joining our programs.  All decisions on scholarships are made based on demonstrated need and are kept entirely confidential.  Please complete the form below.  

If you have more than one player in our program- complete only one form for your family.  When you have multiple players, please complete the name, grade, etc., for the oldest player and indicate other players in question 8.

Please note that this form is only applicable to Boys Lacrosse for full-time residents of Montclair, NJ.

If you answered No to the free/reduced lunch question above-   Please provide a short explanation of why you need a scholarship for your son. (Three to five sentences maximum.) If you are requesting a payment plan please let us know how much of a deposit you are able to make at this time. If we have any questions or require additional information, we will contact you.  All information submitted will be kept confidential.
