                                    Important General Information
Howard University’s financial aid programs recommend assistance in the form of scholarships and loans. Descriptions of these programs may be found on the internet at http://www.howard.edu/financialaid/information.htm. Most financial aid recommendations are made to students who demonstrate that their financial resources are insufficient to meet the cost of attending Howard University.  Certain scholarships however may be awarded solely on the basis of academic excellence. Priority in awarding institutional financial aid funds will be given to eligible students on a first‐come, first‐served basis. However, scholarships may be awarded to students throughout the school year based on need and academic performance. All recommendations are subject to availability of funds and the final determination of student eligibility.
The submission of this application does not guarantee approval for financial aid. Financial aid awards will be made only to
admitted students. Applications from candidates for readmission will not be considered until the request for readmission has been approved. Financial aid awards may only be used to pay educational costs incurred during the period covered. Awards may not be used to satisfy past or future indebtedness to the University or to pay enrollment fees and deposits for University housing. Students must apply annually to be considered for financial aid.
Applicants for federally‐supported programs must be eligible U.S. citizens or permanent residents of the United States.
Permanent residents will be required to submit evidence of this status. In addition, all financial aid recipients must be making Satisfactory Academic Progress (SAP) as determined by Howard University.
The primary responsibility for financing a medical education rests with the student. Applicants are encouraged to explore all outside sources of assistance in meeting their educational costs.
                      Student Responsibilities in Receiving Financial Aid
HONESTY
It is imperative that students applying for and receiving aid be completely truthful in all information submitted on the
application. Any changes in family financial circumstances after submission of application documents should be brought to the attention of the Financial Aid Office. A student is required to report to the Financial Aid Office any financial aid received from sources not previously known by the office.

AWARD ACCEPTANCE
A student accepting all aid offered need not return the award notification. However, a student may decline, in writing, or on line through BisonWeb (http://www.howard.edu/bisonweb/), any or all financial aid awards that are recommended.

Financial Obligations
A student who is delinquent in meeting financial obligations to Howard University will not be allowed to register for the next term or to graduate. It is also the policy of Howard University that transcripts or other certifications will not be issued for students who are financially delinquent. Personal checks will be honored for tuition and fee charges (excluding graduation fees) except from students who have previously submitted checks that have been returned because of insufficient funds.

                            Some Notes Pertaining to the Application
Complete all items on the 2016‐2017 Howard University Health Sciences Application for Financial Assistance. If a particular section does not apply to you, then enter “NA” or select the “NO” option in all necessary fields. However, do note that if you elect not to complete the application as it applies to your health discipline your application will be deemed incomplete.

1. Complete and submit the 2016‐2017 Free Application for Federal Student Aid (FAFSA) online at www.fafsa.gov on or before February 15, 2016. Applicant and the applicant’s spouse (if applicable) must sign this application to be considered for financial aid. Please make sure that you answer the question regarding college name, city, state, and school code as follows: Code 001448, Howard University, 2400 Sixth Street, NW, Washington, DC 20059.

2. Students who wish to be considered for Federal Scholarships for Disadvantaged Students and Loans or Primary Care Loans must complete the parent(s) as well as the student (and spouse) sections on the 2016‐2017 FAFSA. The parental data requirement cannot be waived. Parents’ signatures must be provided on the FAFSA. All application information will be verified. If parents reside outside of the United States, allow extra time to obtain information and signatures.  According to the Student Financial Aid Guidelines established by the Department of Health and Human Services, an individual from a disadvantaged background is defined as someone who:

• Comes from an environment that has inhibited the individual from obtaining the knowledge, skill, and abilities required to enroll in and graduate from a professions or nursing school (Part 1); and/or

• Comes from a family with an annual income below a level based on low‐income thresholds according to family size published by the U.S. Bureau of the Census, adjusted annually for changes in the Consumer Price Index, and adjusted by the Secretary of Health and Human Services for adaptation to this program (Part2)

3. A Student Aid Report (SAR) will be sent to you by the U.S. Department of Education after the FAFSA has been processed. Review the SAR and, if necessary, make corrections online at www.fafsa.gov.

4. All students, including students who previously borrowed from the Federal Stafford Student Loan Program, will be considered for the Federal Direct Student Loan Program. Do not submit a Federal Stafford Student Loan application. No application is needed for the Direct Student Loan Program other than the FAFSA. Your eligibility will be determined by the Office of Financial aid and, if eligible, the Direct Student Loan will be awarded to you.

5. The Office of Financial Aid reserves the right to review and adjust award recommendations at any time.

6. Permanent residents should submit a clear copy of both sides of your Permanent Resident card to the Office of Financial Aid in the College of Medicine. U.S. citizens born outside the United States should provide documents evidencing your citizenship status, such as a copy of a current U.S. Passport or original Certificate of Naturalization.

7. Maintain records of all debts that you incur for your education together with the terms of each loan such as interest rates, deferment options and repayment provisions. Retain a copy of each promissory note signed.

8. Primary Care Loans. Students who are willing to contractually commit to primary care training and practice may contact the Office of Financial Aid for information on low‐interest (5%) Primary care Loans. Primary Care is generally defined as family practice, general internal medicine, and general pediatrics.

9. Credit History. All students should note that private educational loans and Federal GradPLUS loans are only approved for students with acceptable credit histories. We encourage all students to request and review a copy of their credit report (www.annualcreditreport.com). The University cannot meet the financial need of students who do not qualify for student loans because of a poor credit record.

10. Some FAFSA filers will be selected for a process called verification. Students affected will be asked to provide additional information.

Section A:

Enter all of the appropriate classifications.

For questions 9, 10 and, 11 select the ethnicity and race with which you identify. These three questions are designed to identify your ethnicity and race for statistical reporting only. Regardless of your answer to questions 9 and 11, you must answer question 10.

For question 10, please select the racial category or categories with which you most closely identify. Select as many as apply. If you choose to identify as multiracial, please check the multiracial category in addition to the appropriate other racial categories.

For question 12, please provide the expected amount of outside scholarships, fellowships, grants, and/or outside loans (i.e., financial aid not applied for via the FAFSA or this application).

For question 13, please provide your current total educational indebtedness including: Subsidized and Unsubsidized Direct Stafford Loans, Graduate PLUS loans, Primary Care loans, University Loans, and Private Student loans. This amount should not include your expected financial aid for the upcoming academic year. You can locate your aggregate indebtedness at NSLDS (http://www.nslds.ed.gov/nslds_SA/). Section B:

For question 14-15, provide information in this section for your Non-custodial parent(s) or guardian(s). If this circumstance does not apply, please insert "NA" or "0" in the respective fields. A non-custodial parent is the parent with whom the child(ren) do not live with the majority of the time or is the parent that has custody a smaller portion of the time.

For question 19, enter the city and State/Province of the urban or rural area where you have mostly lived in, if applicable.

For questions 20-21:

To determine if you are from an economically disadvantaged background follow the steps below:

1. Access this website (http://www.melissadata.com/lookups/addressverify.asp) and Enter a Street Address, City & State OR Street Address and ZIP Code.

2. Afterward click “Submit.”

3. Scroll down and confirm that the page states “Address Verified.” If it does not, try a different address entering method.
Next to “Census Entities” record the “Tract” number.

4. Next access this website (http://hpsafind.hrsa.gov/HPSASearch.aspx) and Select the appropriate state, county and discipline (i.e. "Medicine" for Medical Students, "Dental" for Dentistry Students, etc. Pharmacy students please select "Primary Medical Care").

5. Afterward click “Show me the HPSAs.”

6. Scroll down the table and search under “HPSA Name’ for your Tract number. If you find your number, then you live in a disadvantaged area and must input the Tract number under question 21.
