Parent Survey for 6-12 Math [17 - 18]
Please complete one survey per family – unless your experience is significantly different at different grade levels. . Please evaluate each of the following items in your child’s math program. Then consider the actual situation in his or her school and indicate your response, by selecting the appropriate number for each response. When considering the scale, 1 means “Not Important” , 3 means "Important" and 5 means “Highly Important” for the importance question, and 1 means “Does Not Happen”, 2 means "Rarely Happens, 3 means "Usually Happens ", 4 means "Mostly Happens" and 5 means “Frequently Happens” for the accomplishment question.
