Fan Panel Application Form 2017
Please fill out this form to the best of your ability. We will open the first round of applications on December 1st. They will close on January 15th. We will notify panelists of their acceptance by January 15th. This first round of panelists will receive priority with scheduling.

Please list everyone involved with this panel in alphabetical order. If you have 4 or more hours of panels accepted, only the first 3 people listed will receive badges. Please include your own name at the top of the list. A person can be involved in multiple group's panels, but they will only receive a free badge if their name appears in the first 3 people for each panel.

Preferred Length of Panel. Please check all the lengths you are willing to make this panel/adjust it to. Being flexible with the length of a panel will allow us to find better places in the schedule for it. As a new policy, all panels will have 15 minutes of set up time before their panel. Please let us know if you will need an additional 15 minutes (total 30 minutes of set up time.)

Preferred Panel Location. We have a variety of sizes and types of rooms. Please choose the one that is most appropriate for your panel.
Large lecture room (300+ people, most likely main events)
Medium lecture room (150-300 people, largest normal panel room)
Small lecture room (0-150 people, smallest panel room)
Workshop room (0-40 people, perfect for classes or crafts)
Large empty room (150-300 people, for events like Cosplay chess that need space to move around)
What kind of equipment will you need?
Prize Tickets- small amount (for lecture style panels)
Prize Tickets- medium amount (enough for panels with a lot of audience interaction)
Prize Tickets- large amount (for game shows, panels with an audience winner.)
