Before you fill out this Evaluation form, you will need to confirm with your CE student the number of hours that they completed at your site. You will also need to confirm with them whether they are continuing at your site for the 2nd quarter of this academic year. You should also discuss your assessment of their performance with them. Students who plan to continue with you for the 2nd quarter will benefit from your feedback and their performance will improve as a result. This evaluation form must be submitted no later than Thursday, October 18th.*IF YOU ARE THE MENTOR FOR MORE THAN ONE CE STUDENT, YOU WILL BE ALLOWED TO COMPLETE A NEW FORM FOR ANOTHER CE STUDENT ONCE YOU HIT SUBMIT.
