HPS Transportation Request Form
Hudsonville Public Schools (HPS) provides pupil transportation as a privilege to students who:

1. Reside within the district boundaries
2. Reside within the attendance area of the school in which the student is enrolled
3. In accordance with Board of Education Policy

Returning students eligible to receive transportation to/from their primary home address will be assigned to a bus stop by the Transportation Department upon completion of this form. Parents/guardians can choose school bus transportation to and or from their primary home address, and to and or from one (1) daycare/alternate site based upon eligibility and a regular schedule. Notifications containing bus information for the 2021/22 school year will be sent out by email and school messenger by mid August.
