I give permission to Uintah Band, Uintah School District, and Uintah High School to use my student's picture(s) on the internet. This may include social media posts and the band web site. *
I give permission for my student to be transported by school bus to all activities pertaining to the the marching band schedule. *
In the event that transportation is needed locally (In the Basin) I give permission for my student to ride with another student as long as the driver holds a current Utah drivers license. *
