Eligibility
Is your school a non-profit public education agency that serves transition-aged students with disabilities? *
Does your school district have a need for improvement as indicated by student, school, and/or district data? *
Has your school submitted the most recent October 1 federal special education census and/or is it serving students with disabilities? *
Has your school applied for current year IDEA Basic Entitlement funds? *
What is the total number of students enrolled in your high school/district? *
What is the total number of students receiving special education services in your high school/district? *
If your school/district has had a team that previously participated with STMP/CCRTT, how many years has it been since they attended? *
Team Membership
The grant award requires the participation of up to five staff members who must be part of the on-site high school staff and includes: 1. School/district administrator or special education administrator or designee, 2. Transition coordinator/specialist or special education teacher, 3. General education teacher, CTE teacher, or guidance counselor. Will your district or charter be able to meet these participant requirements? *
If you answered "Yes" to the previous question, please list the name and job title of all 5 potential candidates. (Note: submitting names at this stage does not commit the staff identified to participating in the STMP/CCRTT, but will expedite the application process once you formally apply for the grant.)
