MS Planned Absence Notification
Please use this form to let us know of any planned absences such as family vacations, etc. Please submit this form as far in advance as possible. Please reference pages 54-55 of the Family Handbook regarding absences and make-up work policies. Please download and print the Student Planned Absence Form for your student to take to his/her teachers.
Note: This form is not required for traditional medical/dental appointments. Students are allowed to miss up to six days of school per semester. There are no distinctions between excused or unexcused absences.

Additional Steps: 1. Download and print the Student Planned Absence Form.  2. The middle school student should use the form to go to each of his/her teachers to acquire their signatures and assignments. If the student is involved with extracurricular activities, then he/she should provide this form to that supervising adult for a signature. 3. The middle school student should turn in the form to the school office in a timely manner (at least two days before the proposed absence). The receptionist will forward the completed form to the principal. A final confirmation notice will be sent to identify the absence status. 
A copy of your responses will be emailed to the address you provided.
