This form allows you to refer a group you have trained to us if they are interested in an Automated External Defibrillator (AED).  You can utilize this form to send us their information and we will contact them directly and email them a direct proposal.  If a purchase results from your referral, you will earn a $50 referral fee or store credit - your choice!  

PS: We will specifically not offer them training if you prefer to continue to provide the training - just check that box option below and we will make sure we do not offer them any training services.
