Wavier of Liability

I hereby acknowledge and understand that participating in sports, programs, and/or events organized by the Chinese Christian Church of Grace in Malden (CCCGM) involves inherent risks, including, but not limited to, accidents, injuries, illness, or even death. I voluntarily assume all risks associated with participation, whether they are known or unknown to me, including falls, contact with other participants, and any other potential hazards.

Having read this waiver and being fully aware of these risks, I, on my own behalf and on behalf of anyone entitled to act for me, agree to hold harmless, waive, and release CCCGM, its officers, employees, and volunteers from any and all claims, liabilities, demands, or causes of action arising out of my participation in CCCGM sports, programs, and/or events.
