To be considered for approval, this application AND supporting documentation MUST be submitted NO LATER than August 1, 2025.

If you are requesting open enrollment for more than FIVE (5) students, please complete a second application.

Open enrollment is a privilege and may be revoked at any time as determined by Lakewood Local School District.

Please note that you must apply for Open Enrollment EACH YEAR and approval is based on student/teacher ratios and other criteria stated in Administrative Guideline 5113. Qualified applicants will be admitted based on building, grade level, and program capacity. The Superintendent's office will notify you in writing of approval or denial of this request.

FALSIFYING ANY INFORMATION WILL RESULT IN DENIAL OF THE OPEN ENROLLMENT REQUEST.

TRANSPORTATION: As stated in Administrative Guideline 5113, Lakewood Local School District is NOT responsible for providing bus service to students attending under the Open Enrollment Policy.

No student shall be denied admission to the Lakewood Local School District or to a particular course or instructional program or otherwise discriminated against for reasons of race, color, national origin, gender, handicap, or any other basis of unlawful discrimination.
