1. Academic Integrity Check Form PU/LIB/FORM/010:

3. Your thesis/article/project in (PDF or Word format)
4. Your supervisors email address(s) where applicable
5. Intent to Submit Letter
6. Acceptance of intent to submit letter from Dean SGS
