INFORMED CONSENT AGREEMENT

The purpose of this survey is to collect data to investigate the effect of COVID-19 (also known as Coronavirus) on mobility and equity.  This study is conducted by Dr. Mansoureh Jeihani and Dr. Celeste Chavis at Morgan State University. Any information obtained in connection with this study that can identify you will remain confidential.  Your decision whether or not to participate will not prejudice your future relations with Morgan State University. If you decide to participate, you are free to discontinue participation at any time without prejudice.
