Please read and complete this form:

The Parents/Guardians/Carers shall:
1. Ensure that my child attends School regularly, on time and properly equipped, and to inform the Academy about reasons for absence before 9.00am on the first day of absence;
2. Avoid absences from school particularly during the period of tests/assessments (dates for which will be notified at the beginning of the new school year)
3. Avoid taking holidays during term time (the Academy and Local Education Authority have the right to fine parents who take their children out of school without approval (see prospectus);
4. Let the Academy know about any concerns or problems that might affect my child's work or behaviour;
5. Support the Academy's policies and guidelines for behaviour;
6. Support my child in homework and other opportunities for home learning;
7. Attend Parents' Evening and discussions about my child's progress;
8. Get to know about my child's life at the Academy;
9. Support the Academy policy for uniform and appearance (including hairstyles) and ensure that clothing is clearly named;
10. Not engage in Facebook or any other social networking site to make negative comments about the Academy or staff.

The Academy will:
1. Encourage your child to do his/her best at all times;
2. Hold a Transition Evening towards the end of the Summer Term to offer the opportunity to meet teachers and give details of curriculum content for the following year;
3. Inform you of your child's progress at regular meetings and by an annual written report;
4. Let you know about any concerns or problems that affect your child's work or behaviour;
5. Provide appropriate homework on a regular basis in accordance with the Academy's behaviour policy;
6. Contact you if there is a concern with the well-being of your child;
7. Contact you if there is a problem with your child's attendance, punctuality or equipment;
8. Seek the views of Parents/Guardians/Carers through questionnaires and meetings;
9. Keep you informed about Academy activities through regular letters home, newsletters, notcies, website and text messaging.
