As a volunteer at Gardening the Community during this COVID-19 pandemic, in order to protect yourself and others, it is critical that you confirm that in the past 24 hours you have not experienced any symptoms of a potential infection, nor have you been in contact with anyone who has been experiencing symptoms. These symptoms may include, but are not limited to:

• Fever (temp of 100+ degrees)
• Chills
• Cough, shortness of breath, or other respiratory issues
• Body aches
• Sore throat
• New loss of smell or taste
• Fatigue

In addition, all volunteers must follow these guidelines:

• Bring and wear your own work gloves
• Maintain a 6ft or greater distance between yourself and others when possible
• Wear a mask or other face covering at all times when a 6ft or greater distance is not possible
• Work in groups of 5 or fewer people
• Work only with staff or other volunteers, NOT YOUTH

In compliance with the government of Massachusetts' reopening guidelines, all farm site work safety protocols will be posted and must be strictly followed.

In consideration of permission to participate in volunteer opportunities of Gardening the Community and TerraCorps (referred to afterward as The Organizations):

I fully understand and acknowledge that outdoor activities have inherent risks, dangers, and hazards and such hazards may exist during my participation.

Participation in such activities may result in injury or illness and I hereby assume all risks, dangers, and responsibility for any losses or damages, whether caused in whole or in part by the negligence of the officers, staff, or agents of The Organizations, the negligence of the participants, the negligence of others, accidents, the forces of nature, or other causes.

I voluntarily agree to release, waive, discharge, hold harmless, defend, and indemnify The Organizations and their officers, staff, and agents from all claims, actions, or losses for bodily injury, property damage, or otherwise, which may arise out of my participation in The Organizations' activities.

I have read the above waiver and release, and by signing it agree. It is my intention to exempt and relieve The Organizations from liability for personal injury, property damage, or otherwise caused by negligence or any other cause.

I consent that any narratives, film, photographs, videotape, or sound recordings of events in which I may be participating may be used by Gardening The Community and TerraCorps and their sub-grantee partners, or the AmeriCorps program, including TerraCorps, the Massachusetts Service Alliance, the National and Community Service Board, or any other AmeriCorps programs in the state, in order to publicize AmeriCorps, Massachusetts Service Alliance, or TerraCorps events, programs, and service day activities, or in any other ways they see fit.
