CCC Chinese School TA Application Form

递交该申请，若被录用，则申请人及其监护人负有如下责任：
1.申请人要遵守学校制度，遵守纽约首府华社中文学校学生助教职责，认真完成指导老师分配的任务。
2.申请人及其监护人应了解纽约首府华社中文学校学生助教职责并支持协助教师助理的工作。

学校收到申请后会与申请人及其监护人电话联系，请确保表格中所填写的电话号码及电邮地址为正确真实的信息。一经录用，新学年开始之前，申请人及其监护人需填好纸质申请表的签名页，并交给中文学校统一存档。

请申请人确认：申请人自愿申请担任中文学校助教，此表格需由申请人亲自填写，不能由父母或他人代劳。 *
