The FC Tulsa Academy Scholarship Program was developed to provide players and their families the
opportunity to receive a scholarship from the club during times of financial need. The program provides
scholarship funds to help offset part of the expenses associated with playing soccer, which may include funds to help
offset the monthly training fees. The Scholarship Program
does not offer funds to cover Registration fees, Administration fees, Uniforms, and/or Team Fees.
FC Tulsa Academy has limited funds available for scholarships. Scholarship applicants must first meet the
financial qualifications before being considered for a scholarship. Scholarships will be awarded based on
availability of funds. There is no guarantee that qualified applicants will receive scholarship monies.
Approval of a scholarship mandates the player is committed to the club for the entire seasonal year, will
participate in all games, practices, scrimmages, and team activities, unless school or religious events
conflict. Additionally, the player will participate in all team/club fundraisers. Our monthly training fees are a
one-time fee that we give our families the option to payout over 12 months. Player will owe the full
amount of the scholarship if the player quits, or seeks a release from the club, before the seasonal year is
complete.
Scholarship Guidelines: Household income should be below the Reduced Price Meals (school meal plan)
level per the U.S. Department of Agriculture to be considered for a scholarship.

To Apply:
1. Submit the questions below.
2. Email a copy of the front page (showing names of dependents at the top and your Adjusted Gross Income at the bottom) of your most recent income tax return to
blitzacademyfc@gmail.com, and list the players name that's applying for the scholarship in the subject line of the email. The tax return must be sent for your scholarship application to be considered. Please mark out all social security numbers as all applications will be handled in a confidential manner.
3. Check with your coach, or team manager, to verify that your scholarship has been accepted if you have not heard from the office by mid-Summer.
Scholarship Application Deadline: Completed applications must be emailed by June 1st to be
considered.
NOTE:
-Please be prepared to bring registration money to tryouts. Once approved, you will see the Scholarship applied to your online account. If you need assistance, please reach out to your coach/team manager. FC Tulsa Academy cannot function without the help of many volunteers.
-Scholarship recipients will be expected to volunteer a minimum of six hours during the soccer year. You may work at our tournaments, or other events. Please reach out to your manager for assistance in scheduling these hours. Falling behind on team dues will put any future scholarship applications in jeopardy.
-If you have additional circumstances that you would like the committee to consider (Example:
unemployment, excessive medical bills, circumstances have changed since last tax return), please include that in an email to blitzacademyfc@gmail.com
