Hello!

Headway currently cannot support supervisory or incident-to billing however it is something we hope to be able to support in the future. This form is strictly to gather interest by current practices utilizing Headway's services AND practices that would be interested in utilizing Headway's services pending Headway could support with supervisory and/or incident-to billing in the future.

Please complete the following form if you are interested in billing supervisory sessions through Headway in the future.
